The main character, Daniel Garner, was deceived in both Heaven and Hell, as a result of which he remained in purgatory forever. Its events take place after the plot of the supplement for the original game and skip all subsequent logical sequels, but there is an explanation for all this, which is revealed in the ending.

This is the seventh game in a row from the entire 'Painkiller' series, which is a sequel-remake of the first part of the 2004 release. 'Painkiller: Hell & Damnation' is a Polish first-person shooter.
